ER Admissions & Transfer Center Management

Transfer Center Screening
EHR Physicians work with case management to screen all transfers into the hospital using medical necessity criteria and economic indicators. EHR Physicians work with the transferring and accepting physicians to ensure appropriateness of transfers from both a financial and clinical perspective.

Key Elements of the Transfer Center Screening Program:

Preserves the hospital's ability to act as "gatekeeper" when appropriate.

Removes the attending physician from the role of "bad guy", which can negatively impact referral relationships.

Allows the hospital to avoid inappropriate and unreimbursed transfers.

Emergency Department Admission Screening
EHR Physicians work with case management in the emergency department to apply medical necessity criteria to all new admissions. When an admission is not medically necessary, EHR Physicians work with the admitting and emergency department physicians to ensure adequate documentation to justify admission, or help create a plan of care that avoids an unreimbursed admission.

Key Elements of the Emergency Department Admission Screening Program:

Provides consistent medical management oversight of the admission process.

Avoids payor denials or custodial admissions before they occur.

Educates treating physicians on documentation and facilitating plans of care.

Improves hospital throughput by ensuring admissions are admitted to the appropriate level of care.
